2. Surname
3. Date of Birth (DOB)
4. Mobile Number
5. Email Address
6. Postal address
7. Course Name (BSc Optometry or Ophthalmic Dispensing Degree)
8. Year of Study first, second, third, fourth
9. Your TU Dublin Student ID number

 Additionally, please review and confirm your agreement to the Consent to Levy document.
